job_description.job_card.job_descriptionSupport the clinical care coordinator and the overall success of the program. Order and maintain program supplies; collect, maintain, and report pertinent patient data points. Schedule and prepare patient education classes; coordinate patient appointments within the hospital. Answer program phone calls and web inquiries; navigate patients through the referral process. Provide outstanding customer service and maintain respectful relationships with patients and team members. Serves as a liaison between the hospital, physician practices, and the patient to ensure optimal customer and clinical service and effective communication. Advocates for patients; sets up appointments and services within the hospital and assists with travel arrangements as needed. Navigates program inquiries, gathers patient history, medical records, and demographic / insurance information for referrals. Accesses patient records to obtain program metrics; enters data, tracks trends, and supports quality improvement projects. Coordinates logistics for pre-operative education classes and events. Contributes to program development by assessing patient needs and supporting service improvements; participates in marketing strategy activities toward service goals. 3 years of experience providing high-level customer service (Required) 1 year of experience with navigating electronic medical records (Required) Associate degree in healthcare, sociology, or related field (Preferred) Additional software continuing education and training (Preferred) Previous secretarial experience (Preferred) Experience in clinical aspects of the medical field (Preferred) Formal customer service training (Preferred) Experience working in a hospital or physician office (Preferred) Benefits from Day One Paid Days Off from Day One Debt-free Education (Certifications and Degrees without out-of-pocket tuition expense) For eligible positions Career Development Whole Person Wellbeing Resources
