What Youll Do :

Carry out rehab plans under a licensed PTs guidance for homebound patients

Work directly with clients on physical improvement goals using progressive, evidence-based techniques

Deliver functional training to promote fall reduction and restore independence

Document treatment sessions and patient responses for team insight and ongoing care adjustment

Engage with interdisciplinary team members to ensure wraparound support for every client

Qualifications :

Current Physical Therapy Assistant license in Minnesota

Access to a vehicle, valid license, and insurance

Passion for in-home care and commitment to client progress

Open to experienced professionals and newer grads seeking meaningful roles
