Are you a compassionate, experienced RN who loves making personal connections and helping patients understand their options at a vulnerable time? As a Hospice Admissions Nurse, you’ll be the face of our care—helping families say “yes” to compassionate end-of-life support when it matters most.
What You'll Do as a Hospice RN Admissions Nurse :
Report directly to the Admissions Manager and serve as a key ambassador for our hospice servicesVisit patients and families in their homes, hospitals, or facilities to assess eligibility for hospice careExplain hospice philosophy and services clearly, compassionately, and with clinical confidenceGuide patients and their loved ones through the admissions process, ensuring a smooth and supportive start to careCollaborate with referring physicians, case managers, and hospital or facility staff to coordinate transitions of careUse the nursing process to assess patient needs, support care planning, and document clinical findingsBuild trust and provide emotional reassurance to patients and families as they make this important decisionAccurately complete required documentation and ensure compliance with state and federal regulationsWork closely with the clinical and interdisciplinary team to ensure timely admissions and quality care handoffAbout You :
Qualifications - What You’ll Bring :
Active RN license in the state of employment (or eligible to obtain)1+ year of general nursing experienceA hospice heart : compassionate, empathetic, and deeply committed to patient-centered careStrong assessment, communication, and interpersonal skillsComfortable visiting patients in hospitals, skilled nursing facilities, or home settingsValid driver’s license, auto insurance, and reliable transportationCurrent CPR certificationPreferred Experience (Not Required) :
2+ years of hospice, home health, healthcare admissions, or medical salesBachelor’s Degree and / or Certified Hospice and Palliative Nurse (CHPN) certificationBackground in case management, care coordination, or healthcare outreachExperience with electronic medical records and compliance documentationWe Offer :
