About the Role

Rising Ground’s Health Services team seeks a Nurse Practitioner to provide advanced clinical care to youth and adolescents engaged in our programs. The NP will work within 29i services, which deliver integrated healthcare to young people involved in foster care, residential treatment, and juvenile justice programs—populations with significant and diverse health needs.

What You’ll Do

• Perform health assessments, diagnoses, and treatments across our Bronx and Brooklyn clinics.
• Prescribe medications and develop treatment plans in line with NYS regulations.
• Collaborate with care teams to align medical and behavioral health strategies.
• Promote preventive care and health education for youth and their families.
• Ensure compliance with healthcare regulations and standards.
• Document clinical care accurately in the Electronic Health Record.

Work Environment :

Based at the 29i clinic locations in Brooklyn and the Bronx, with regular travel between sites. Work is conducted in clinical settings within program facilities.
