Care Coordinator

A company is looking for a Care Coordinator to serve as a patient advocate and point of contact for care recommendations.

Key Responsibilities

Support patient experience preadmission and throughout the continuum of care

Ensure timely follow-up with patients and facilitate internal transfers between sites

Develop working relationships with interdisciplinary team members to ensure effective communication regarding patient care

Required Qualifications, Training, and Education

Bachelor's degree preferred; High School diploma required

Two years of customer service and relevant experience required, preferably in a healthcare setting

Experience with computers and proficiency with software platforms and EMRs

Knowledge of medical / psychiatric / chemical dependency diagnosis and treatment processes preferred

Valid driver license preferred and must be granted insurable status by the insurance policy (Oconomowoc only)
