Job Qualifications :

Education :  High School Graduate

Experience : 2-3 years Home Health intake experience preferred, LVN candidates accepted

Skills :  Required computer proficiency and excellent interpersonal, organizational, and phone skills. Must be able to move at a fast pace.

Functions :

• Receives and processes new referrals in a timely manner.
• Facilitates care coordination with sales reps, referral sources, physician offices, patients, and agency clinical leaders.
• Effectively communicates and relays accurate care coordination details to internal and external constituents in the referral coordination process.
• Takes ownership of recording accurate information on the referral tracker and in the EHR and ensuring all referrals move through the process quickly and responds and communicates in a professional manner.
• Demonstrates commitment, competency, and ability to handle a lot of details proficiently.
• Exhibits Reliant at Home's Bluebird Promise.
