Job Summary :

The purpose of the Case Manager I position is to support the physician and interdisciplinary team in facilitating patient care, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction while managing the cost of care and providing timely and accurate information to payors. The role integrates and coordinates utilization management, patient education, care facilitation, and discharge planning functions. The Case Manager I is accountable for a designated patient caseload and plans effectively to meet patient needs, manage the length of stay, and promote efficient utilization of resources.

13 week contract REQUIRED : Minimum 5 years acute inpatient case management experience with experience in acute discharge planning, BSN, BLS certification through AHA, Previous experience using CarePort system. Candidates with previous experience charting in AllScripts, GEMS / Sunrise are preferred. Need candidates who are willing and able to be on the floor with patients and who can acclimate very quickly. Overtime is billed after 48 hours $140 bill rate will only apply while the Grand Blanc strike is active. Rate will decrease to $80-85 when the strike ends.
