About the Role :

The Licensed Master Social Worker (LMSW) at the Henryetta Hospice Branch plays a critical role in delivering compassionate and comprehensive psychosocial support to patients and their families facing life-limiting illnesses. This position is responsible for assessing the emotional, social, and environmental needs of hospice patients, developing individualized care plans, and coordinating with interdisciplinary teams to ensure holistic care delivery. The LMSW advocates for patient rights and access to community resources, facilitating communication between patients, families, and healthcare providers to promote dignity and quality of life. This role requires a deep understanding of grief counseling, crisis intervention, and ethical decision-making within the hospice care context. Ultimately, the LMSW contributes to the mission of providing empathetic, patient-centered care that supports both patients and their loved ones through challenging end-of-life experiences.

Minimum Qualifications :

• Master’s degree in Social Work (MSW) from an accredited program.
• Current and valid Licensed Master Social Worker (LMSW) credential in the state of Oklahoma.
• Minimum of one year of clinical social work experience, preferably in hospice, palliative care, or related health care settings.
• Strong knowledge of hospice regulations, psychosocial assessment techniques, and counseling methodologies.
• Ability to pass background checks and maintain compliance with all healthcare regulatory requirements.
• Valid Driver License and auto insurance
• AHA CPR Certification

As a Social Worker, you will :

Assesses and completes the Comprehensive Assessment within 5 days of the patient’s election of hospice care to identify patient and family’s psycho social, financial, environmental and community needs as evidenced by POC documentation, clinical records, IDG meetings and community resource referrals.

Assists with advance directives, funeral planning, discharge planning, family counseling and may assist in changes in the level of care as indicated by the patient or family’s needs.

Use effective interpersonal relations and communication skills to achieve desirable outcomes.

Provides patient and family / caregiver counseling around issues of end-of-life, death, and grief.

Attends interdisciplinary team meetings, participates in the patient care planning process and coordinating and collaborating with the IDG to promote coordination of patient care.

Provides an environment which promotes respect for patients and their privacy and property.
