job_description.job_card.job_description2-3 years social work experience. Previous recent inpatient behavioral health experience. Ability to perform intake assessment. Experience leading groups. Experience charting in EMR / EMH system. Preferred : EPIC charting experience. Candidates will be required to have weekend availability. Graduation from an accredited school of social work with a master’s degree. Current LMSW certification.
Location : Jackson, MI 49201 (Onsite)
Role Type : W2 contractor role
Schedule : Mon-Fri, 8am-5pm
Duties
- The Social Worker works with the psychiatrist and interdisciplinary team in developing and managing treatment plans.
- Serves as liaison in relation to patient care and needs, between the hospital, community, and insurers to facilitate access to clinical treatment for individuals.
- Provides social services to both adult and child / adolescent patients.
- Services include but are not limited to bio-psychosocial assessments, discharge planning, individual and family education and group.
- Therapy Social Workers provides individual, conjoint, group and family psychotherapy; in addition to crisis intervention services, as needed.
- Ensures that the quality of care is safe, effective, patient centered timely, efficient and equitable.
- Social workers also serve on designated committees and as qualified, may serve as field instructors for social work.
