Care Manager (RN, MSW, OT, PT, ST) – Part-Time & Full-Time Lexington, KY area
Diversified Life Care Partners (DLCP) is expanding and seeking experienced, compassionate Care Managers to join our Aging Life Care™ team. We are hiring for both part-time and full-time positions, giving you the flexibility to match your schedule with your career goals.

What You’ll Do
- Conduct comprehensive, in-home assessments to identify client needs, risks, and goals
- Develop and implement personalized, whole-person care plans across health, social, financial, and life domains
- Serve as a true advocate — supporting clients in healthcare, community, and personal life settings so they can live fully and independently
- Coordinate services such as home care, rehab, transportation, enrichment, and specialty providers
- Guide families through crises and transitions with compassion and clarity
- Document assessments, recommendations, and outcomes in our secure platform
- Collaborate with families, physicians, attorneys, financial professionals, and community partners to ensure client-centered care
What We’re Looking For
Active license : RN, MSW, OT, PT, or ST10+ years of relevant experience (geriatrics, home health, hospice, case management, or social services)Strong understanding of aging issues, chronic conditions, and senior care resourcesExcellent communication, organization, and problem-solving skillsCompassionate, client-centered approach with ability to work independentlyReliable transportation; ability to travel across Louisville, Lexington, and Southern IndianaComfortable with documentation, email, and mobile scheduling toolsWhy Join DLCP
