About The Position

Job Title : Registered Nurse

Hours per Shift : 8 hours (08 : 00 - 17 : 00)

Experience : Minimum 1 full year of adult home health experience or 1 full year of hospice experience depending on the specialty.

License : Must have a valid nursing license.

Certifications : CPR, BLS

Must-Have : Proficient in HomeCare HomeBase (HCHB) and OASIS within the last 6 months.

Description :

Provides, coordinates, and directs the provision of home nursing care according to physician orders based on applicable policies and procedures.

Responsible for the competent application of the nursing process.

Supervises and evaluates home health aides and licensed practical nurses.

Travel is required for home visits and periodic office meetings.

Must have reliable transportation, a current driver's license, and agency-required liability insurance.

Expected to see between 5-10 patients per day, equating to approximately 6-8 productivity points daily.

Documentation must be completed within 24 hours of each appointment.

Clinician must accept all scheduled patients regardless of location.

No guaranteed hours are provided in this contract.

Must be willing to travel up to 75 miles for patient visits.

Mileage will be reimbursed at the standard IRS rate.

Local candidates are accepted at the same rate.

Experience with HomeCare HomeBase EMR is required.

Candidates must be open to potential on-call rotations or weekend work depending on the schedule.

All travelers must have recent specialty experience.
