Position Overview

As an Occupational Therapist (OT) , you will evaluate, plan, and implement treatment for residents using evidence-based approaches to improve independence and quality of life. You’ll also play a key role in patient and family education, interdisciplinary collaboration, and documentation of care.

Key Responsibilities

  • Evaluate residents to determine functional impairments and therapy needs.
  • Develop and implement individualized treatment plans focusing on ADLs and IADLs.
  • Monitor patient progress and adjust treatment as needed, collaborating with the interdisciplinary team.
  • Recommend adaptive equipment and home modifications to promote safety and independence.
  • Educate patients, families, and caregivers on techniques and strategies to support functional recovery.
  • Maintain thorough and timely documentation in accordance with state and federal guidelines.
  • Attend required meetings and actively participate in care planning.
  • Ensure compliance with all regulatory and facility policies.

Qualifications

  • Active Texas Occupational Therapist license
  • In good standing with all relevant licensing boards and agencies
  • Experience in Skilled Nursing Facilities (SNF) or long-term care preferred
  • Familiarity with Net Health documentation system is a plus
  • Strong interpersonal skills and the ability to work with diverse patient populations
