SUMMARY
The Community Outreach Worker plays a vital role in advancing HSI’s mission to reduce community violence and promote holistic well-being through trauma-informed, community-centered engagement.The incumbent will connect directly with individuals and neighborhoods most affected by violence, working to prevent harm, build trust, and create pathways to support.
QUALIFICATIONS
- High school diploma or GED
- Must be available to work weekdays, occasional nights and weekends.
- Three (3) years of violence prevention, trauma-informed care, and community engagement strategies experience working with inner-city or high-risk youth in a social service or community-based setting.
- Proficient in technology use, including data entry and case tracking systems.
- Must pass background and child abuse / neglect registry checks.
- Valid Driver’s License and access to reliable transportation
RESPONSIBILITIES
Lead and coordinate targeted street outreach efforts in assigned communities to prevent and respond to violence.Conduct community canvassing, relationship-building, and crisis response to incidents of violence.Build trust with individuals at risk and assist them in accessing support services through trauma-informed care approaches.Collaborate with Case Managers and Prevention Specialists to support individualized service plans and goals.
