JOB SUMMARY
The Physical Therapist works under the supervision of their Clinical Supervisor and the Rehab Director. He / she is responsible to organize and conduct medically prescribed physical therapy programs to restore function and prevent disability following disease, injury, or loss of a body part; helps patients reach their maximum performance and assume a place in society while learning to live within any limits for his / her capabilities.
PRIMARY JOB DUTIES
Performs patient assessment and documents in medical record.Develops an age appropriate plan of care, which addresses physical, psychosocial and spiritual needs, in collaboration with other members of the health care team.Implements the plan of care, delegates appropriately and ensures documentation of interventions.Sees all patients as scheduled and assists with other patient needs throughout the continuum if needed.Completes daily treatment notes on time that provide support for continuation of skilled therapy.Completes and turns in daily all notes, charges, and other paper work.Evaluates progress towards desired outcomes, revises plan of care, and intervenes as necessary.Educates patient and families regarding current health status, plan of care, and anticipated outcomes.Provides service excellence to all customers.Demonstrates efficient time management and prioritization skills.Maintains professional standards related to clinical practice, staffing and continuing education.Practices fiscal responsibility and accountability.Works required work hours and participates in weekend coverage to meet patient needs. Is flexible during times of high and low census and will provide coverage to other CCH rehabilitation service lines as needed.Demonstrates positive interpersonal relations.Maintains a clean and safe working environment that is conducive to high quality care.Adheres to CCMH policies and procedures as well as the WYOT statutes governing the supervision of PTAs.Complies with the hospital’s Corporate Compliance Program including, but not limited to, the Code of Conduct, laws and regulations, and hospital policies and procedures.Must be free from governmental sanctions involving health care and / or financial practices.Performs all other duties as assigned.JOB SPECIFICATIONS
Education
Graduate of an accredited physical therapy schoolLicensure
Must be licensed in, or be eligible to be licensed, in the State of WyomingCertifications required
