Illinois Licensed Palliative Care Nurse Practitioner

A company is looking for a Palliative Care Nurse Practitioner (IL-Licensed).

Key Responsibilities

Conduct specialty palliative care outpatient appointments via video visits as part of an interdisciplinary team

Oversee interdisciplinary case conferences to create and monitor individualized care plans for members with advanced serious illness

Provide education on palliative care to Community Health Workers and Oncology Nurses, focusing on cancer-related symptoms and advance care planning

Required Qualifications, Training, and Education

At least 3 years of nurse practitioner experience, with a minimum of 2 years in palliative care

Advanced certified hospice and palliative nurse practitioner (ACHPN) certification

Unrestricted nurse practitioner license with a willingness to obtain additional state licenses as needed

Experience in an outpatient oncology setting is preferred

Familiarity with technology and virtual care platforms
