(LPN or RN) MDS Nurse

Edith Lane of Cincinnati is seeking a compassionate, detail-oriented MDS Nurse (LPN or RN)  to join our dedicated team. This full-time role is responsible for coordinating and overseeing the  Minimum Data Set (MDS)  assessment process to ensure accurate and timely documentation that supports quality resident care and optimal reimbursement.

Key Responsibilities :

• Coordinate and manage the  Resident Assessment Instrument (RAI)  process in compliance with federal and state regulations.
• Complete and submit MDS assessments accurately and on schedule.
• Collaborate with interdisciplinary team members to develop and update individualized care plans.
• Monitor resident status and ensure assessments reflect current clinical conditions.
• Educate staff on MDS processes and documentation best practices.
• Participate in quality assurance and performance improvement (QAPI) initiatives.
• Maintain up-to-date knowledge of Medicare / Medicaid reimbursement guidelines and regulatory changes.
