Division and Program Description

Felton’s Senior Division specializes in a full continuum of mental health services for older adults in outpatient clinics, the community, and supportive housing sites, as well as programs in employment training, economic security, case management, advocacy, and social connection and inclusion. The Geriatric Outpatient Services and the Intensive Case Management programs are part of Felton Institute’s Mental Health Model for Older Adults, specializing in client centered, recovery-oriented treatment for individuals living with severe psychiatric disorders. Services encompass a continuum of care addressing client's goals and needs, which may include medical and substance abuse co-morbidities, a history of homelessness, significant social isolation, and case management needs. Under a contract with the Department of Public Health's Behavioral Health Services (BHS), the multidisciplinary team includes a psychiatrist, psychiatric nurse practitioner, clinical case manager, and peer / community specialist. Services are city-wide and include extensive outreach, therapy, and case management to reduce risk of an individual's psychiatric hospitalization and to support the individual living successfully in their community. The program collaborates with the broad spectrum of social service providers to connect, coordinate, and meet the individual's needs. Felton is committed to implementing evidence-based practices and behavioral health research within its community-based mental health services.

Job Description

Under supervision of the Program Director, the clinical case manager participates in a multidisciplinary team, including master's level clinicians, peer specialists, interns, and psychiatric medication providers, serving as the lead point of responsibility for enacting client care. The clinical case manager maintains a caseload serving clients in both the Geriatric Outpatient Services (GOS) and Intensive Case Management (ICM) programs. The GOS caseload is capped at a higher level to allow for long term services, while the ICM caseload is capped at a lower level to allow for intensive services to stabilize individuals who are experience acute symptoms and related risk factors. Once individuals achieve stabilization, clients are referred for lower levels of care, including outpatient services and / or primary care as clinically appropriate. The GOS / ICM clinician evaluates needs of clients, and develops comprehensive clinical assessments, treatment plans, and coordinates care among the team. Services may include individual and group therapy, crisis intervention, case management, client advocacy, and engagement strategies. The GOS / ICM clinician also maintains accurate detailed records of client contacts through Cal-AIM (Medi-Cal) and Medicare standards as well as county and state regulations. Additional duties as needed.

Specific Responsibilities

  • Work closely with interdisciplinary team.
  • Provide crisis intervention to prevent hospitalization, and if necessary, facilitates involuntary hospitalization.
  • Perform intakes, assessments, and service plan development for older adults with psychiatric and dual diagnosis disabilities in a Felton clinic, community, and / or in their homes.
  • Provide clinical case management, which includes individual or group therapy, case management, connection to primary care, and coordination with other providers.
  • Maintains clinical records of each client contact or service in accordance with mandated standards of Medi-Cal, Medicare, and County Behavioral Health Services, and meets requirements of timely access for client care, documentation, and billing requirements.
  • Participates in team meetings and case conferences through presentation of cases, discussion of clinical issues and exchange of information and ideas.
  • Commits to ongoing training, including developing specializations in evidence-based treatments and practices and the geriatric specialization of mental health services, as provided by the Senior Division and Felton Institute.
  • Other duties and responsibilities as assigned.

Minimum Requirements and Qualifications

  • Master's degree or Licensed in Social Work; Marriage and Family Therapy, or Professional Counseling and registered with active license with CA BBS, including ASWs, AMFTs, and APCCs.
  • A minimum of 2 years of case management and mental health treatment for at-risk older adults, including co-occurring disorders (master’s level internship experience considered)
  • Moderate to comprehensive understanding of Specialty Mental Health Services under the Department of Health Care services definitions, in addition to skills in assessment, treatment planning, and mental health services pertaining to individuals living with serious mental illness
  • Strong recovery model approach to care and ability to form strong therapeutic relationships with clients from diverse backgrounds and who often are underserved by traditional care models
  • Ability to effectively diagnose and make clinical rationales for psychiatric care utilizing the DSM 5 and other measures including social determinants of health and functional impairment screenings where appropriate
  • Actively practices cultural humility and promotes a warm and welcoming environment for culturally and gender diverse individuals
  • Excellent communication skills both in written and verbal formats
  • Ability to travel within SF to meet clients in a variety of community settings including SROs, hospitals, shelters, and other settings required
  • Ability to be a team player with a welcoming attitude

    FLSA Status : Exempt

    Salary Range : $90,000 - $105,000 Annually (DOE)
