• Conduct comprehensive patient assessments to develop personalized care plans that address individual needs.\n

• Educate patients and their families on health management, medication adherence, and lifestyle modifications.\n
• Collaborate with physicians and other healthcare professionals to coordinate care and ensure optimal patient outcomes.\n
• Document all patient interactions, interventions, and progress in medical records in compliance with HIPAA regulations.\n
• Participate in discharge planning to facilitate smooth transitions from hospital to home care settings.\n
• Provide support for patients with diverse conditions including geriatrics, pediatrics, infectious diseases, and behavioral health challenges.\n
• Engage in continuous professional development to stay updated on best practices in home health nursing.\n\nBasic qualifications\n
• Valid Registered Nurse (RN) certification / license in the state of Illinois (IL).\n

You will be part of a supportive team, providing essential care and coordination for patients and their families.\n\nKey Responsibilities : \n\n? Manage patient care in hospice settings\n? Coordinate with healthcare teams\n? Provide leadership and support to nursing staff\n\nSchedule and Shift Details : \n\n? Monday to Friday, 8 AM to 5 PM with varying start and end times. On-call duties will rotate monthly, averaging around five shifts per month.\n\nTravel : \n\n?
