Summary :
Responsibilities :
1. Utilizes and records the nursing process as delegated by the Registered Nurse.
Collects and documents selected data.
Admission process and ongoing assessments are based upon collection and interpretation of data relevant to health status of patient. Appropriate forms are completed. Patient problems are identified and supported by assessment data. Patient assessments are documented within 8 hours of admission and early in the shift. Assists with planning patient care individually taking into account his / her limitations and / or specific requirements.
Patient calls are answered promptly
2. Participates in the promotion, maintenance, and restoration of health, provides comfort measures for a dignified death.
Practices clinically competent and caring behavior
Responds to patient requests for comfort measures promptly. Nursing care is provided by incorporating the principles of pain management, medication administration, nutrition, skin and airway integrity, elimination and mobility. Problems and changes in patient's condition are reported to Charge Nurse / MD as they occur. Interventions and patient responses are accurately documented and reported throughout the shift. Patient is able to verbalize plan of care. All orders are completed for the shift.
Completes medication and treatment passes in a timely manner and in accordance with hospital policy.
3. Implements care that is legal, ethical and respectful of patient's rights.
Respects and maintains patient's right to privacy and confidentiality.
Patient information is communicated only to those identified as aiding in care. Patients are informed of their rights and their questions are answered. Charge Nurse is informed during shift of ethical concerns patient expresses, and guidelines are followed as instructed. Care is provided to each patient in a nonjudgmental and nondiscriminatory manner.

4. Provides a safe, secure patient environment.
Follows Safety and Infection Control Principles.
Universal precautions and isolation procedures are followed. Hazardous waste is properly disposed. Safety precautions are implemented. Medication and IV calculations are accurate. Nursing care is provided in accordance to age and corresponding stages of growth and development.
5. Collaborates with other health care team members in providing care.
Maintains accountability for duties accepted, including care given by self and by others to whom care is delegated.
Patients' follow-up care process is identified prior to discharge. Plan of care is developed and implemented with aid of case managers and other members of health care team. Assignments accepted from the Registered Nurse are completed utilizing established policies, procedures and practices. Duties delegated to nursing assistants are within their scope of practice.
Provides and receives report upon beginning and ending shift.
6. Acquires and maintains current knowledge in nursing practice.
Seeks knowledge and skills needed for the practice setting.
Ongoing educational activities related to clinical knowledge and professional issues are maintained yearly. Maintains current licensure to practice. Participates in house wide and / or unit teams or committee task forces. House wide / unit continuing education offerings are maintained and current.

7. Demonstrates a positive attitude and a courteous, professional image at all times.
Communication with co-workers and manager are harmonious. Patients' surveys will reflect satisfaction with care received.
8. Performs duties with attitude in accordance with hospital policies and philosophy.
Ensures unit / hospital cost containment contribution through efficient use of time and resources. Utilizes appropriate Safe Patient-Handling equipment, where indicated, to ensure personal patient safety.

Current licensure to practice as a Licensed Practical Nurse in the state of North Carolina. BCLS Certification.
