Monitor and report changes in client conditions, including reactions to treatments or medication. Educate and support clients and caregivers in managing health at home-a core responsibility in high-quality RN Case Manager roles. Document all services accurately and promptly using Vivie's EMR system. Understand payer sources and authorization requirements. Complete and document supervisory visits with LPNs and HHAs in compliance with regulatory guidelines. Lead and organize skilled nursing care aligned with state and federal standards, Vivie's policies, and professional best practices in senior care. Deliver the highest degree of quality care and customer service with fiscal responsibility. Maintain compassionate, clear communication with clients, families, physicians, referral sources, and interdisciplinary teams. Use exceptional communication skills to ensure continuity of care across all positions on your team. Participate in team meetings, assist with projects, or support other areas as needed. This ensures our clients always receive the high-quality care they can count on. The duties and responsibilities listed above are representative of the nature and are not necessarily all inclusive. Current Registered Nurse (RN) license in the state of Minnesota. CPR certification (required within 6 months of hire). Valid driver's license and access to an insured personal vehicle. Minimum 1 year of previous home health nursing experience . Ability to pass state-mandated background check. Proficiency in English. Physical ability to perform job duties, including driving to client locations and hands-on care. Employment Type : Hourly, non-exempt (eligible for overtime) Department : Nursing Leadership Received : Regional Clinical Manager Division : Home Health Travel Requirements : Travel to client homes within designated regions. This role includes supervisory visits with LPNs and HHAs but does not include formal supervisory responsibilities.
Schedule : Full Time - 64-80 hours / pay period (0.8 - 1.0 FTE - You Pick!) - Monday to Friday - 8 a.m. to 4 : 30 p.m.
