Position Highlights
A Brief Overview
Serves as a patient liaison to help patients and family members "navigate" the complex healthcare systems from time of suspicious lesion to diagnosis. This position works with a multidisciplinary team providing improved comprehensive breast health services to patients. Requires expertise to identify and implement improvement processes, and the ability to coordinate and effectively communicate breast care for Avera customers. Internal and external contacts include nursing staff and management, physicians, patients, families, community agencies, and professional peers.
What you will do
Works closely with patients and physicians in multiple specialty areas to coordinate patient's care and appointments. Communicates with all members of the healthcare team on behalf of the patient. Coordinates care from suspicious lesion to diagnosis with providers and scheduling for efficient turnaround of additional imaging and biopsies.
Assists patients and their families in obtaining physician orders or referrals. Provides support and counseling, as appropriate to the clinical situation. Provides and remains knowledgeable of options for imaging funding, social services and shares breast care resource options to optimize outcomes.
Understands Clinical Care and Protocol related to research - developing and maintaining working knowledge with breast health related clinical research protocol criteria. Assists with patient assessment during invasive procedures and post care for procedures in the Avera Breast Center as needed. May perform breast imaging in the area individual is credentialed in.
Proactively develops and enhances processes with the goal of improving the clinical experience for referred patients and the referring physician. Collects and evaluates follow up data related to breast patients in order to provide information for ongoing quality improvement.
Communicates with referring physician's offices as appropriate to the patient and physician's needs. Assists in coordinating discharge planning and resource referrals.
Assists with and coordinates breast health community speaking and educational opportunities. Partners with community agencies and organizations to promote breast health for women in our region. Remains clinically informed of current trends, and procedures related to breast care. Facilitates support group meetings as needed. Assists with education needs for organizations staff. Assists with coordination of breast conferences as needed.
Performs follow up on abnormal findings for appropriate care and required documentation. Performs various duties as needed to successfully fulfill the function of the position.

Required Education, License / Certification, or Work Experience :
Associate's Graduate of an accredited program in radiologic technology
ARRT Certification - American Registry of Radiologic Technologists (ARRT) An active license in the state of practice Upon Hire or

Mammography (M) - American Registry of Radiologic Technologists (ARRT) Upon Hire or

Bachelor's in Radiologic Technology
Breast Health Clinical Navigator (BHCN) - National Consortium of Breast Centers within 2 Years
Experience in breast imaging

Evaluates patient care by adhering to therapeutic standards; measuring health outcomes against patient care standards; reviewing patient records; noting changing conditions and needs; re-evaluating care objectives; modifying nursing care plan.
Maintains safe, secure, and healthy patient care environment by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations; adhering to organization standards and policies, and to legal regulations; alerting others regarding potential concerns.
Documents patient care services by charting in patient and department records.
Keeps patient care equipment operating by following operating instructions and established procedures; troubleshooting breakdowns; maintaining supplies; performing preventive maintenance; calling for repairs.
Keeps patient care supplies ready by checking stock; anticipating needs; placing and expediting orders; verifying receipt; stocking items.
