  • Using resources judiciously and effectively.\n\nThe RN, Coordinator oversees and coordinates the delivery of nursing and medical assistant services at the Stout Street Health Center or a satellite clinic. This role ensures high-quality, trauma-informed care by leading nursing operations, developing staff, and facilitating interdepartmental collaboration. The position combines administrative leadership with direct clinical responsibilities to support optimal health outcomes for individuals receiving care through the Coalition.\n\nNOTE : In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance. Please note that a start date will be scheduled once credentialing and privileging is complete.\n\nAdditional Requirement\n

  • Ensure high-quality, trauma-informed nursing care, including oversight of support staff.\n
  • Manage and develop a team of professionals by providing supervision, coaching, performance evaluations, and professional development opportunities. Foster a supportive team environment that promotes accountability, collaboration, and continuous improvement.\n
  • Supervise Registered Nurses, Medical Assistant Supervisors, and Medical Assistants including hiring, orientation, scheduling, and performance management.\n
  • Coordinate daily clinical schedules and maintain appropriate staffing ratios for RNs and MAs.\n
  • Champion and lead quality improvement initiatives utilizing LEAN methodology and coach team members in best practices.\n
  • Ensure timely & accurate recording of all clinical / non-clinical services, communications, and tasks in the EHR and other systems as required while maintaining complete up-to-date charts and any future upgrades as directed.\n
  • Support and oversee RN-delivered patient education focused on chronic and acute health conditions, including diabetes, hypertension, wound care, gender-affirming care, family planning, and disease prevention.\n
  • Ensure that RNs perform rapid and accurate triage assessments both in-person and via telephone, in alignment with CCH nursing protocols and trauma-informed care standards.\n
  • Oversee and support RN responsibilities related to OBH-licensed SUD treatment, including MAT counseling, withdrawal assessments, and care coordination.\n
  • Facilitate mentorship opportunities and preceptorships for new nurses and nursing students in collaboration with RN staff.\n
  • Serve as a resource for problem-solving, team communication, and coordination across integrated care teams.\n
  • Oversight of the program, including but not limited to, creating policies and procedures, materials management, lab integrity and medication storage management.\n
  • Perform other job duties as assigned.\n\nQualifications Summary\n
  • Minimum of 2 years of supervisory experience in a clinical setting, preferably in a community health, public health, or long-term care environment.\n
  • Minimum of 4 years of experience as a Registered Nurse.\n
  • Ambulatory, home health, urgent care, or public / community health experience preferred.\n
  • Strong critical thinking, crisis intervention, and decision-making skills.\n
  • Ability to lead and support trauma-informed, patient-centered care in a high-volume, fast-paced setting.\n
  • Understanding of and sensitivity to issues affecting people experiencing homelessness or underserved populations.\n
  • Registered Nurse licensed in the State of Colorado.\n
  • Associate's or Bachelor's degree in Nursing (BSN) required.\n
  • Master's in Nursing or Healthcare Leadership preferred.\n\n$77,792 - $91,520 a year\n\nWHERE A CANDIDATE IS PLACED IN THE COMPENSATION RANGE DEPENDS ON TOTAL RELEVANT YEARS OF EXPERIENCE\n\nEmployee must be able to perform essential job functions with or without reasonable accommodation and without posing a direct threat to safety or health of self or others.
