We are seeking a Family Practice Physician for a full-time and permanent position in Colebrook, NH. The ideal candidate will have at least 2 years of experience in outpatient primary care, preferably in a rural health clinic, and must be Board Certified. In this role, you will manage a diverse patient panel across age groups, focusing on chronic disease management with no on-call responsibilities.

Family Medicine Opening in Colebrook, NH -Full Time & Permanent Position.-Experience required. Job Description -Provider must have minimum of 2 years in practice in outpatient primary care, rural health clinic experience preferred.-Able to work autonomously.-Must be Board Certified.-Office location is in Colebrook NH.-Direct patient care time will be split between acute care and established patient panel.-Replacement for provider that is retiring. Primary Care : -Pediatric through Geriatric.-High elderly population.-Experienced in Chronic Disease Management.-16-22 patients per day.-36 patient hours, 4hrs of admin time - NO CALL.-EMR is Meditech Expanse, Dragon dictation.-Supported by an RN Team leader and medical assistant.
