About The Position

Job Title : Hospice Social Worker

Profession : Social Work

Shift : Day Shift

Hours per Shift : 8 hours

Experience : Minimum of 1 year of experience, preferably in hospice care

License : LCSW

Certifications : BLS

Must-Have : Hospice social work experience

Description :

Census of 30 patients in home / facility.

4-5 visits per day, with scheduling autonomy for social workers.

Patient types primarily include end-of-life care, with no pediatric patients.

Patient ratios are approximately 1 social worker to 35 patients.

Two other social workers in different markets provide additional support.

Collaboration with the hospice medical director is expected.

Familiarity with electronic medical records, particularly EPIC, is preferred.

Weekend rotations are not required.

There is no on-call schedule.

1-2 holiday commitments are based on unit needs.

Requests for time off (RTO) can be up to 5 days.

Shift times are Monday through Friday, from 8 : 00 AM to 4 : 30 PM.

Scrub color is flexible, with an option for business casual attire.
