Slate Healthcare is seeking a travel nurse RN Case Management for a travel nursing job in Rapid City, South Dakota.

General Information

Minimum Experience : 1.5 years general nursing (system requirement)

Will you accept a first-time traveler? Yes, only if greater than 2 years of experience

Travelers must live >

100 miles away from any Monument Health facility

Permanent staff must wait 1 full year to return as a traveler

Hours : 0800-1630

Patient Types :

Case Managers provide care for patients in all the acute care departments

The RN Case Manager will most likely remain in the same area during their contract, pending new hires. Typically, travelers are not required to float to multiple areas during their contract.

Patient Ratios :

1 CM to 16-20 patients

Required Certifications :

BLS

Education Required :

Completion of a nursing education program that is approved by a board of nursing;  Registered Nurse (RN) - South Dakota Board of Nursing

Skills required :

Previous discharge planning and Acute Care Case Management experience, progression of care, experience meeting with families and discussing plans of care

Support on the Unit :

Case Management Assistants Lead Case Managers

Social Work – Complex Psychosocial patient consultants

Utilization Review RN’s Hospitalist

Charting System : EPIC (required)
