Schedule : Mon - Fri, 9 : 00 AM - 5 : 00 PM (with on-call rotation)

Job Overview :

We are seeking a dedicated Physician to provide medical care for patients in Long-Term Care (LTC) and Transitional Care Units (TCUs) in the Hudson / St. Croix Falls, WI area. The provider will float between these locations, ensuring high-quality, patient-centered care for residents in long-term and post-acute care settings.

Key Responsibilities :

• Perform comprehensive initial and ongoing assessments of LTC and TCU patients.
• Develop and implement individualized care plans based on medical history and patient needs.
• Provide routine and urgent medical care, including diagnosing and treating acute and chronic conditions .
• Prescribe medications, order diagnostic tests , and adjust treatment plans accordingly.
• Collaborate with an interdisciplinary team (nurses, therapists, social workers) to optimize patient care.
• Educate patients, families, and caregivers on treatment options, medication adherence, and disease management .
• Ensure documentation is accurate and compliant with regulatory standards.
• Address end-of-life care decisions and collaborate with palliative and hospice teams when needed.
