Licensed Practical Nurse (LPN) – School-Based 1 : 1 Role

\n Location : Westford, MA | Pay Rate : $35 / hr | Full-Time | Onsite

Connected Health Care is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide 1 : 1 support for a student in a school setting in Westford, MA. This is a full-time role during the school day, and includes supporting the student both in the classroom and during transportation.

\n\n

Position Details : \n\n \n\t

Schedule : Monday – Friday, 8 : 50am – 2 : 55pm

\n\t\n\t

Compensation : $35 / hr

\n\t\n\t

Setting : Elementary school, onsite only

\n\t\n\t

Additional Duties : Riding the bus with the student (transport support required)

\n\t\n

\n\n

Medical Services Include :

\n\n \n\t

Taking vital signs

\n\t\n\t

Preparing nutrition, including medical feedings (G-tube, J-tube, bottle, thickening)

\n\t\n\t

Toileting, including colostomy bag management

\n\t\n\t

Seizure monitoring and intervention as needed

\n\t\n\t

Medication administration and documentation

\n\t\n\t

Regular communication and consultation with the school nurse

\n\t\n

\n\n

Educational Support Responsibilities :

\n\n \n\t

Assist with classroom and behavior management

\n\t\n\t

Support implementation of the student’s individualized education plan (IEP)

\n\t\n\t

Observe, collect data, and report to the Special Educator and Classroom Teacher

\n\t\n\t

Provide instruction and guidance in structured settings as needed

\n\t\n\t

Accompany the student to general education classes, specialists, and school activities

\n\t\n

\n\n

Requirements : \n\n \n\t

Active Massachusetts LPN license

\n\t\n\t

Pediatric or school-based nursing experience preferred

\n\t\n\t

Ability to collaborate effectively with teachers, therapists, and families

\n\t\n\t

Compassionate, detail-oriented, and student-focused approach

\n\t\n

\n\n
