Licensed Practical Nurse (LPN)

Affinity Home Care is a boutique concierge agency providing in-home care services throughout New York City and Long Island!

We are hiring compassionate & dedicated LICENSED PRACTICAL NURSES (LPN'S) to provide medical support to our clients.

Duties and Responsibilities :

• Assist with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting.
• Provide direct nursing care to patients in their homes under the supervision of an RN or physician.
• Administer prescribed medications, treatments, and therapies accurately.
• Monitor patient health, vital signs, and symptoms, and report changes promptly.
• Educate patients and family members on proper home care techniques and health maintenance.
• Maintain accurate, detailed, and timely nursing documentation.
• Perform wound care, catheter care, and tracheostomy care as needed.
• Support patient emotional well-being and provide compassionate end-of-life care when necessary.
• Ensure compliance with all healthcare regulations, agency policies, and individualized care plans.
• Assist with mobility and transfers, ensuring patient safety and fall prevention.
• Conduct patient assessments during each visit and update care plans as necessary.
• Participate in training sessions and professional development to maintain nursing licensure and skills.

Requirements :

Valid LPN license in NY (REQUIRED)
