RESIDENCY TRAINED REQUIREMENTS - Residency certificate / Diplomas from accredited University

ATLS, ACLS, PALS, BALS Required? - ATLS, ACLS, PALS required Preeclampsia Certificate.

CALL REQUIREMENT - None

EMR SYSTEM - EPIC

ACTIVE STATE LICENSE REQUIRED TO PROCEED? - Yes, DEA needs to be pending, active DL / passport, Permanent Resident card

FLU OR OTHER IMMUNIZATIONS REQUIRED? WAIVER CONSIDERED? - Flu within 12 months waiver is considered. MMR and or Titer, TB within the past 12 months

PRIVILEGING TIMEFRAME - 90 days

EXPEDITED LOCUM PRIVILEGES? - No

ONBOARDING REQUIREMENTS (Orientation / EMR training) - New / Returning provider orientation, EPIC EMR training, needs to be done within timeframe.
