Description
Astrana Health is seeking a Licensed Behavioral Health Counselor (LCSW / LMFT) to provide telehealth services to Medicaid members experiencing substance use disorders (SUD) and co-occurring behavioral health (BH) diagnoses. This is a unique opportunity to manage an acute caseload of patients who cannot receive timely follow-up care following a hospitalization for SUD or BH admissions. Your goal will be to provide timely and thorough follow-up care to patients who are high utilizers of the ER with intent to link these individuals to longer-term, sustainable solutions in managing their diagnoses.
