About The Position

Certifications : CPR, BLS

Must-Have : Proficiency in HomeCare HomeBase and OASIS

Description :

Provides coordination and direction for the provision of home nursing care according to physician orders.

Approach care based on agency policies and procedures through the competent application of the nursing process.

Responsible to Clinical Manager.

Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.

Home visits and periodic office meetings are required.

Must have reliable transportation and a current driver's license.

Must carry agency-required liability insurance.

Average number of patients per day : 5-8.

Contractor will be assigned between 5-10 patients per day.

Each type of visit and activity (documentation, travel, etc.) holds a different point value.

Guaranteed hours are not provided.

If patient census is low, contractors can discuss floating to another clinic to pick up hours.

Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.

No first-timers accepted; recent specialty experience is required.

Weekend requirements vary by location and profession; please discuss during the interview.

On-call requirements vary by location and profession; please discuss during the interview.

Territory mileage radius : Must be willing to travel up to 75 miles.

Mileage reimbursed at the IRS Standard rate.

Local candidates accepted at the same rate.

Pending licenses are not accepted; candidates must have a license in hand.

Must be open to traveling up to 75 miles away from assigned care center.

All documentation must be completed within 24 hours of each patient appointment.

Delayed and incomplete documentation that impact billing and follow-up patient care may result in a cancellation.

Candidates must accept every patient on their schedule regardless of distance from their previous appointment.

No guaranteed hours are provided with this contract.

Weekly productivity points of 30 are expected, equating to approximately 40 hours per week.

Traveling candidates should be comfortable with longer drives between patients.

Must meet the minimum experience requirement and be open to on-call rotation.
