Program : Amarillo, Texas – PESC Continuity of Care (CoC)
Salary range :
Case Manager I : $40,809.60 Annually
Case Manager II : $43,305.60 Annually
Case Manager III : $46,009.60 Annually
Shift available : Full-time

Essential duties and responsibilities :
- Staff will maintain ethical and professional communications at all times using respectful Trauma-Informed Care language
- Part of TPC's Rural Mobile Mental Health Team and will provide routine case management, skills training, psychosocial rehabilitation, treatment planning, assessment, & care coordination services to clients in their own communities, homes, or other location of the client's choosing
- Travel across the 21 counties of the Texas Panhandle to assist and manage a caseload of people who have been recently discharged from psychiatric hospitalization or at-risk of hospitalization
- Travel is required within TPC's 21 county catchment area daily to provide treatment services. Our regional clinics are in Borger, Clarendon, Dumas, Hereford, Pampa, & Perryton.
- Work alongside other TPC providers in the rural areas to identify clients most in need of intensive services. Some of these clients will be returning to the rural communities after psychiatric hospitalization and are in need of ongoing mental health treatment and support.
- Focus on learning rural resources and providing outreach to improve access to care
- Visit hospitals to meet with clients, establish care plans, and continue providing care after the clients are discharged and move back into their community
- All services will be provided using evidence-based practices that are person-centered and trauma-informed
- Need to have the skills necessary to build rapport and establish trusting relationships with clients right after a crisis situation
- Introduce clients to a range of services offered by TPC and other community services and will provide care coordination to facilitate access to these resources
- Participate in multidisciplinary team meetings and case staffing's to develop treatment approaches that best fit the needs of the client
- Maintain same day next day documentation requirements and a 95% or higher compliance average for all individuals served
- Other essential functions and tasks necessary to carry out the purposes of the position will be required
- Adheres to TPC Business Code of Conduct and reports concerns if ethical and / or business standards defined in the code are compromised
Minimum qualifications to be successful in job :
Bachelor’s degree from an accredited university with a major in psychology, sociology, social work, counseling, or other related human services field for QMHP credentialingFor Case Manager I, no experience requiredFor Case Manager II, minimum one (1) year of experience in behavioral health case managementFor Case Manager III, minimum two (2) years of experience in behavioral health case managementGraduate work in a related field may be substituted for up to one (1) year of experienceEmployees may be required to utilize their personal mobile phones for two-factor authentication (2FA).
