Develop, implement, and monitor individualized care plans for residents in collaboration with medical staff, families, and caregivers. Act as a liaison between residents, families, healthcare providers, and community resources to facilitate optimal care and services. Train, lead, and support a team of case managers and related staff to maintain high standards of care. Monitor residents’ progress and make necessary adjustments to care plans to ensure their needs are consistently met. Maintain accurate and up-to-date documentation and reports related to case management activities. Address and resolve any concerns or issues related to resident care in a professional manner
