JOB SUMMARY
The RN Case Manager for Home Health provides nursing care in accordance with the client's plan of care, and includes comprehensive health and psycho-social evaluation, monitoring of client's condition, health promotion and prevention, coordination of services, teaching and training activities and provision of direct nursing care and other duties as deemed necessary.

EXPERIENCE AND EDUCATION
Required
Graduate of accredited nursing program.
2 years of RN experience with one (1) year experience with adult IV expertise required.
(RN) REGISTERED NURSE by Board of Nurse Examiners for the State of Texas (or BNE Compact State). and
(DRIVERS) DRIVERS LICENSE Valid Texas driver's license, proof of driver's insurance. and
(BLS) BASIC LIFE SUPPORT accredited course by the American Heart Association (AHA) or American Red Cross (ARC)
Preferred
Bachelor's Degree - BSN
Other RN experience including high tech and home health expertise preferred.
Prefer knowledge of InterQual and TMF care criteria.
