Openings : 1 full-time provider, but open to a part-timer who can work a minimum of 15 hours / week
Setting : Outpatient
Location : Uniontown, PA 15401
Schedule : Currently, Mon- Fri 10am-3pm, but looking to scale to Mon- Fri 9am-4pm & Sat 9am-2pm
Pay Rate : W2 : $73.33 | 1099 : $84.62

- Population : adults 18+; 80% of patients are seen for substance use
- Minimum requirement : must have certifications, open to new grads with prior medical exp like an RN
- Interview process : 1 remote interview with Dr, 1 on-site interview with a provider
- Have physicians to provide collaboration
- Support staff : office manager (background in nursing), PAs, NPs, and Physicians
- EMR System : Tebra

Summary
This role involves providing comprehensive primary and specialty medical care, working collaboratively with our team in a clinical setting in Uniontown.
Responsibilities
Patient Care and Assessment : Conduct patient histories, perform physical examinations, and assess presenting illnesses, risk factors, and psychosocial situations.Diagnosis and Treatment : Order and interpret diagnostic tests, develop and implement treatment plans, and diagnose both acute and chronic conditions.Medication and Procedures : Prescribe and administer medications and injections in accordance with state laws and practice guidelines. Perform minor in-office procedures and manage wound care.Patient Education : Instruct and counsel patients and their families on disease management, preventive care, and healthy lifestyle choices.Collaboration : Work closely with physicians and other team members to ensure continuity of care and make appropriate referrals to specialists.Documentation : Maintain accurate and timely electronic medical records for all patient encounters.Qualifications
Education : Advanced degree from an accredited Nurse Practitioner or Physician Assistant program.Licensure and Certification : Current, valid Pennsylvania state licensure as a Certified Registered Nurse Practitioner (CRNP) or Physician Assistant-Certified (PA-C).Prescriptive authority from the state of Pennsylvania.Current Drug Enforcement Agency (DEA) licensure.Basic Life Support (BLS) certification.Skills
Strong clinical judgment and critical thinking skills.Excellent interpersonal and communication skills to effectively interact with patients and colleagues.Ability to work both independently and as part of a collaborative healthcare team.Proficiency in using electronic medical records (EMR) systems.
