job_description.job_card.job_descriptionProvide mobile outreach and engagement Screen and assess for program eligibility Promote positive health outcomes for clients utilizing mobile case management, care coordination with other providers, and supportive services with a team of dedicated professionals Connect clients and their families to a medical clinic and providers, as well as other clinic and community-based services Arrange transportation and accompany clients to office visits as needed Serve as an advocate for the client and family Use motivational interviewing, trauma-informed care, and harm-reduction practices Create and maintain all required documents in the electronic and paper record systems At least one year of experience working with youth strongly preferred Experience providing case management services preferred Experience working with Seriously Mentally Ill (SMI) population is preferred Proficient computer skills, including MS Office A valid California driver’s license in good standing, personal vehicle, proof of insurance and eligibility to be a CRF Approved Driver are required to fulfill the essential functions of this position Ability to pass criminal / background clearance
Hours : 40 hours per week; Monday - Friday, 8 : 00 am to 4 : 30 pm (no on-call, evenings, or weekends)
Pay Rate : $24.90 to $26.21 per hour
Education : High School / GED diploma with 2 years of experience working in social services or related setting required. Bachelor’s degree in Public Health, Social Work, Psychology, Human Services or related field with 1 year of experience working in social services or related setting strongly preferred.
