We are seeking a skilled and compassionate Speech-Language Pathologist (SLP) to join our rehabilitation team in Fort Atkinson, WI. The SLP will assess, diagnose, and treat residents with speech, language, voice, cognitive-communication, and swallowing disorders. This role plays a vital part in helping residents regain communication abilities, improve swallowing safety, and enhance their overall quality of life.
Responsibilities :
Evaluate residents’ speech, language, communication, and swallowing abilities using standardized tests and clinical observation.
Develop and implement individualized treatment plans to address identified deficits and goals.
Provide therapy for speech, voice, language, cognitive-communication, and swallowing disorders.
Instruct residents and caregivers on exercises, strategies, and techniques to improve communication and swallowing safety.
Collaborate with the interdisciplinary team to coordinate comprehensive resident care.
Document evaluations, treatment sessions, progress notes, and discharge summaries accurately and timely.
Participate in care planning meetings and contribute to the development of resident-centered care goals.
Educate staff, families, and caregivers about strategies to support residents’ communication and swallowing needs.
Maintain compliance with all applicable state, federal, and facility regulations and professional standards.
Qualifications :
Current licensure as a Speech-Language Pathologist (SLP) in the state of Wisconsin.
Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA preferred; CFY candidates may be considered.
Experience in a skilled nursing facility or healthcare setting preferred, but not required.
Strong communication, interpersonal, and organizational skills.
