• Work in collaboration with the RN to fulfill the defined patient-specific care plan\n
• Collaborate with the care team to ensure all patient needs are fully addressed\n
• Deliver high-quality skilled care to patients\n
• Thoroughly document care delivery daily in our EMR system\n
• Contribute to a culture of caring through individual accountability and teamwork\n\nSkills for Success : \n
• Compassionate in care delivery, focused on results\n
• Solution-driven, self-motivated, and responds with urgency\n
• Love learning, motivating and inspiring patients to reach their goals\n
• Able to work independently without direct oversight\n

• Familiar and comfortable with technology. HCHB experience is a plus\n\nExperience to Deliver on our Mission : \n
• Current Vocational / Practical Nurse License, valid state driver's license and auto liability insurance\n
• One year experience as a Licensed Vocational / Practical Nurse in a clinical care setting required.
