Description :

Evaluate and assess patients’ physical, emotional, and cognitive abilities to determine therapy needs.

Develop and implement individualized treatment plans to improve daily living skills and independence.

Provide therapeutic interventions including exercises, activities, and adaptive techniques.

Educate patients and families on adaptive equipment, home modifications, and self-care strategies.

Collaborate with physicians, nurses, and interdisciplinary teams to ensure coordinated care.

Monitor and document patient progress and adjust treatment plans as needed.

Ensure compliance with professional standards, state regulations, and ethical guidelines.

Promote patient engagement, motivation, and safety throughout the rehabilitation process.

Participate in staff meetings, quality improvement programs, and continuing education.
