Transitions Care Coordinator

A company is looking for a Transitions Care Coordinator.

Key Responsibilities

Assist care coordination for patients discharged from ER visits and acute care facilities

Act as a liaison between care teams and external medical providers

Monitor admissions and clinical outcomes, providing feedback as necessary

Required Qualifications

At least 1 year of patient-facing experience in a healthcare setting

Preferred experience as a hospital case management assistant or home health care coordinator

Experience working with senior patients preferred

Understanding of Mac iOS and Google Suite

Spanish speaking preferred
