About The Position

Job Title : Registered Nurse

Profession : Nursing

Hours per Shift : 8 hours (08 : 00 - 17 : 00)

Experience : Minimum 1 full year of Adult Home Health experience or 1 full year of Hospice experience, depending on the specialty.

License : Must have license in hand, no exceptions.

Certifications : CPR, BLS

Must-Have :

• Proficiency in OASIS and HomeCare HomeBase within the last 6 months.
• Cannot currently be a direct employee of any home health location if applying as a traveler.
• Must be open to traveling up to 75 miles from the assigned care center.
• Must be open to on-call rotations.

Description :

Provides coordination and direction for home nursing care according to physician's orders.

Responsible for following agency policies and procedures through the competent application of the nursing process.

Supervises and evaluates Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.

Home visits are required, along with periodic office meetings.

Must have reliable transportation, a current driver's license, and agency-required liability insurance.

Average number of patients per day is between 5-8.

Each type of visit and activity, such as documentation and travel, holds a different point value.

The contractor will be assigned between 5-10 patients per day, equating to roughly 6-8 points.

There are no guaranteed hours.

If there are insufficient patients to meet financial needs, the contractor may discuss floating to a nearby clinic for hours.

Types of patient cases include Start of Care, Wound Care, Follow-up Visits, etc.

Candidates must be comfortable with longer travel times, as drives may exceed 20 minutes.

All documentation must be completed within 24 hours of the appointment.

Delayed or incomplete documentation that impacts billing and follow-up care may result in a cancelation.

Weekly productivity expectations are approximately 30 points, totaling around 40 hours per week.
