Job Title : Physical Therapist
Reports to : Director of Rehabilitation Services
Purpose :
The primary purpose of this position is to plan, organize, develop, and provide physical therapy services in compliance with federal, state, and local regulations. The goal is to support residents in achieving their highest level of physical function and independence through high-quality rehabilitative care.
Key Responsibilities :
Program Planning and Development :
Develop and organize physical therapy programs and activities in accordance with nursing care regulations.
Create and maintain physical therapy treatment standards and protocols.
Utilize outcome measurement tools to evaluate and improve therapy effectiveness.
Patient Care :
Review physician orders and schedule therapy services as prescribed.
Conduct evaluations and administer therapeutic interventions based on each resident’s clinical needs and history.
Assist in discharge planning, care plans, and quarterly resident assessments.
Maintain comprehensive documentation including treatment records, resident files, and progress notes.
Quality Assurance and Compliance :
Contribute to the development of quality assurance programs for physical therapy services.
Ensure compliance with all federal, state, and professional practice regulations.
Administrative Functions :
Interpret and explain physical therapy policies and goals to staff, residents, and families.
Ensure therapy charges are accurate and all documentation is completed in a timely manner.
Participate in interdisciplinary meetings and facility committees as needed.
Safety and Sanitation :
Ensure therapy spaces are safe, sanitary, and properly equipped.
Follow facility protocols for infection control and utilize safety equipment when necessary.
Qualifications : Education :
Bachelor’s or Doctorate degree in Physical Therapy from an accredited institution.
Experience :
Minimum of one year of experience in a hospital or nursing care facility is preferred.
Licensure :
Must hold a current, unencumbered Physical Therapist license in the state of practice.
Skills :
Strong verbal and written communication skills.
Ability to work effectively with diverse populations, including elderly and physically impaired individuals.
In-depth knowledge of physical therapy procedures, treatment planning, and therapeutic equipment.
Proficiency in documentation and familiarity with therapy-related software.
Physical Requirements :
Must be able to lift, move, and carry up to 25 pounds.
Ability to stand, walk, and move intermittently throughout the workday.
