Certification & Licensure Requirements

• Certified Peer Recovery Specialist (CPRS) within 2 Years
• Driver's License Upon Hire
• Lived experience with 2 years of uninterrupted recovery.

Required English Skills Advanced reading skills Advanced writing skills Advanced oral skills Communication Skills Ability to respond appropriately to customer / co-worker Interaction with a wide variety of people Maintain confidential information Ability to communicate only the facts to recipients or to decline to reveal information Ability to project a professional, friendly, helpful demeanor Computer Skills Basic computer knowledge : Uses word processing, spreadsheet, e-mail application, and web browser. Comfortable within a Windows OS and learning new applications.

Responsibilities

Ensures Patients / Residents receive comprehensive services : Identification of patient into services, assessment, discharge planning, and other aspects of warm handoff.

Provide continuum of care / follow up care to patients.

Provide education / information to patients, families, and other invested parties.

Assist in facilitation of services, based on quality initiatives and documentation in EMR.

Facilitate Recovery Planning Provide Safe environment for Residents Provide Case Managment to those residing in recovery home

Compensation and Benefits

The compensation range for this position is $15.62per hour - $25.3per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
