Description :
Looking for a Physical Therapist to provide services in home health settings.
The therapist will assess, evaluate, and update the therapeutic, rehabilitative, and functional status of patients.
Participation in the development of the total plan of care is required.
The therapist will evaluate the home environment and assess for muscle strength, mobility, gait, and range of motion.
Directing physical therapy treatment is a key responsibility.
The therapist will instruct patients and their families or caregivers in the use and care of therapeutic appliances.
Identifying priority needs for physical therapy is essential.
The therapist will report the patient's reaction to treatment or changes in condition to the physician.
Initiating physical therapy programs and instructing other personnel or family members is required.
The therapist may train patients in the use of prosthetic devices.
Supervision of aides participating in the patient's rehabilitation is also expected.
Identifying patient and family needs for other home health services and referring as necessary is important.
Clinical and progress summaries based on goal attainment must be prepared and submitted.
The therapist will observe, record, and report information on the patient's condition to the attending physician and in the patient's health record.
Providing consultation to families and caregivers when indicated is part of the role.
Inservice education programs for nursing agency personnel may be needed.
Participation in peer consultation processes and ongoing staff competency assessments is required.
Compliance with regulations and licensure requirements is necessary.
Other duties may be assigned and / or requested.
