A company is looking for a Remote Bilingual RN Care Navigator.

Key Responsibilities

Manage case management specific to kidney health, including member data entry and care coordination

Educate members on kidney health, dialysis, and transplant options while advocating for patient needs

Collaborate with healthcare providers and community resources to develop and maintain member-centric care plans

Required Qualifications

COMPACT, active, unrestricted RN license required

Bachelor's degree required

CCM certification preferred

Three years of experience in case management preferred

Bilingual fluency in English and Spanish
