About The Position

Experience : Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience, depending on the specialty.

License : Must have an active nursing license in hand.

Certifications : CPR, BLS

Must-Have :

Must be proficient in HomeCare HomeBase EMR and Oasis documentation.

Must have reliable transportation, a current driver's license, and agency-required liability insurance.

Description :

The Registered Nurse coordinates and directs the provision of home nursing care according to physician's orders.

Home nursing care is provided based on agency policies and procedures through the competent application of the nursing process.

The Registered Nurse is responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.

This position requires travel for home visits and attendance at periodic office meetings.

The average patient load is 5 to 10 patients per day, with productivity expectations set at 30 points per week.

Visits may include start of care, wound care, and follow-up visits.

All documentation must be completed within 24 hours of the patient visit.

No guaranteed hours are offered with this contract, and clinicians must be open to varying patient census.

Weekly productivity points must be achieved to support financial needs.

Travel may be required up to a 75-mile radius from the assigned care center.

Local candidates are accepted under the same rate conditions.
