Schedule : Full-time | 64-80 hours per pay period | Monday-Friday | 8 am – 4 : 30 pm

Location : Can be based out of any Vivie Home Health location, serving Vivie's Southeast Home Health footprint

PT Liaison Core Key Duties :

  • Serve as liaison between field staff and administration to support the delivery of clinical practices and interventions that drive the organization’s strategic initiatives
  • Host a virtual session for onboarding therapists geared toward PDGM strategy for that specific discipline
  • Share evidence-based research for best clinical practices and / or org strategic initiatives on a quarterly basis
  • Collaboration with Education Manager on the following :
  • Develop discipline specific content for bi-annual discipline specific meetings geared toward enhancing / elevating the care our therapists provide while supporting strategic quality measures
  • Identify quality educational courses as aligned with company needs or practice gaps. Assist in grant rationale write up requests as needed.
  • Partake in steering committees upon request to review discipline specific orientation framework on an annual basis
  • Partake in skills fairs and competency labs as needed
  • Collaborate with Quality, Compliance and Informatics team members on the following :
  • Partake in steering committees upon request to assist in revision of service codes, documentation or process pilots as a field staff representative
  • Attend QAPI on a quarterly basis
  • Maintain communication and relations between other Vivie / Ethos liaisons and full discipline specific team to ensure all feedback is represented
  • Other duties as assigned

Time Commitments : approx. 4-6 hours / month

Education and Experience Required :

  • Must be a current Vivie or Ethos employee.
  • Must be licensed as an Physical Therapist or PTA in the state of Minnesota or North Dakota.
  • Minimum of three years’ experience in practice, one of which must be in the Community Based setting.
  • Must obtain CPR certification within 6 months from date of hire.
  • Must have a valid driver’s license and use of an insured personal vehicle.
  • Must understand the operational excellence, fiscal responsibility, and community based federal and state regulatory guidance.
  • Ability to provide feedback and innovation for new strategies.
  • Must complete and pass a state required background study.
  • Able to communicate verbally and in writing to the extent required by the position.
  • Able to physically perform the duties required by the position.
  • Required to have the ability to read, write and speak English well enough to communicate with staff, clients and visitors.
