Primary Purpose :

The person in this position will be responsible for the overall coordination, direction and supervision of the Residential Program, Individual Home Support or Continuous Residential Support, inclusive of the individuals, staff, facility(s) and all related information. Ensure compliance with all regulations established by all relevant regulatory bodies and policies established by the CCARC, Inc. Board of Directors. The person in this position will be current regarding trends and information as related to providing services to individuals with disabilities, with the goal to maximize the potential of each individual

Essential Functions :

• Supervise and manage the assigned Residential Program in coordination with the Director of Residential Services, to ensure that the standards established by all relevant regulatory bodies and policies established by CCARC, Inc. Board of Directors are met.
• Annually develop the Individual Plan – IP for each individual that identifies, evaluates and implements, with the assistance of the Interdisciplinary team and the person centered planning process, the supports needed by the individuals, including but not limited to, identification of medical and health needs, transportation needs, assistance with community resources and assist in determining that adequate financial / insurance benefits are maintained. Review the plan as needed with the individuals and the team, making adjustments and / or changes as requested.
• Record changes, progres,s and status of the resident’s objectives for all reviews.
• Organize and participate in team meetings within the Residential Programs to include :

individual (resident) meetings

The employee must have fine manipulation ability of both hands and be able to have repetitive movements of the wrists, hands or fingers.

The employee must occasionally to frequently lift, push or pull and / or move 35 pounds, maneuver a manual wheelchair, ascend or descend stairs
