Compensation : $18 per hour. $2 shift differential hours between 7pm and 7am. Schedule : Shifts vary between 4 to 8 hours. 2 : 45pm - 10pm or 5 : 45am - 9 : 45pm. Must have CNA certification and BLS CPR active.

Key Responsibilities

• Accountable for providing care that is based on licensure / certification within the scope of practice.
• Provides care in compliance with the patient's established Plan of Care such as :
• Helps to maintain optimal hygiene and assists in maintaining a safe environment.
• May plan and prepare nutritious meals.
• Assists the patient with ambulation as tolerated.
• Encourage patient independence.
• Performs routine housekeeping tasks.
• Reports any change in the patients mental or physical condition to the Clinical Manager or Director of Nursing.
• Maintains an updated clinical record on each patient at all times, meeting required deadlines for documentation.
• Communicates with caregivers as needed for continuity of patient care.
• Participates in FFHC Quality Assurance, Performance Improvement program (QAPI) as requested.
• Continually strives to improve his / her skills.
• Attends and / or completes all mandatory training / in-services as assigned.
• Submits required personnel file documentation in a timely manner.
• Adheres to FFHC compliance and ethics expectations.
• Maintains patient and family confidentiality.
• Maintains professional, supportive, and responsive interpersonal communication skills.
• Performs other duties as assigned.
