Salary : $62,400 to $72,800 / yr

Hourly-$30-$35 / hr

Job responsibilities can include but are not limited to :

  • Assist the Registered Nurses and Nurse Practitioners in carrying out the plan of care.
  • Assist the in performing specialized procedures (as defined in the State Nurse Practice Act) as directed by the Registered Nurse under physician's orders.
  • Experience using EClinicalWorks (ECW) for Primary Care and Wound Care.

eClinicalWorks provides integrated healthcare IT solutions for medical professionals, including an Electronic Health Record (EHR) system and other tools for patient engagement, population health management, and revenue cycle management. The company's products help with various aspects of a medical practice, from front-office scheduling and patient check-in to provider documentation and back-office billing.

  • Assist with the preparation, implementation and continuing evaluation of the patient plan of care.
  • Assisting the patient with activities of daily living, including the teaching of self-care techniques;
  • Preparing equipment and supplies for treatment that requires adherence to sterile or aseptic techniques.
  • Actively participate with staff in collaborating to ensure the patients plan of care is kept current and any identified issues are addressed.
  • Help reinforce teaching to the patient / family members regarding end of life care.
  • Foster cooperative effort among personnel by understanding the functions of other persons involved in patient care and by active participation in team and staff conferences.
  • Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on the job learning.
  • Prepare clinical and progress notes as appropriate.
  • Keep the Case Manager informed of changes noted during visits.
  • Performs on-call duties (including holidays) per policy and need.
  • Assume responsibility for self-development by continually striving to improve his / her Nursing practice through formal education, attendance at workshops and conferences, active participation in professional and related organizational meetings, and / or individual research and reading. ( Opportunities to earn BSN available.)
  • Qualifications :

  • Graduation from an accredited School of Nursing.
  • Must be licensed in the State of Texas as a Licensed Vocational Nurse or LPN.
  • One or more years of nursing experience.
  • Must be a licensed driver with an insured automobile in good working order.
  • The ability to communicate well in English, both verbally and in writing.
  • Additional Skills :

  • Must be proficient with ECW-eClinicalWorks
  • Proficient with billing for Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) and other programs as required.
  • Experience in Wound Care and Primary Care preferred
  • Training can be provided for individuals who are willing to learn.
